You will be given opportunity to detail your child’s medical conditions on your enrolment form. Should there be any changes to your child’s health, please complete the Medical Conditions & Medication Management form and submit at the office.
If your child is required to take medication at school you must complete and return the Medication Authority form.
If your child has an Asthma Plan please provide a copy from your GP to the office.
